I had a 1.3mm melanoma with no clear margins removed from my left inner thigh area I go to the Melanoma and Breast cancer center after my dermatologist refers me to them and then find out that the nurses giving me the info are not practitioners . I can not understand how having a microscopic ,encapsulated cell in my sentinel node and the other node was clear makes me a stage 4. She told me that if it was found in any of the 2 they removed she would have to move me from a stage 1b to a stage 4 This was before the pet scan was done !!!. I had a pet scan done on Dec. 4th and it came back completely clear! I asked how I could be stage 4 when it hadn't spread to any other areas of my body or distant nodes or organs! She said it was because they found it in my sentinel node ! I had no mitotic index and no ulceration! I am scared to death and have yet to be seen by an oncologist because my nurse says that they won't treat me until after my doctor goes back in to remove 3 or 4 more nodes although my doctor told her to make the appointment for me to see oncology and he even thought I had already been seen by one! I have seen my doctor only once since my surgery and he believes I am going to be just fine and says that I am going to live a very, very long time. I feel like he tells them to do something and they ignore him and do what they want to do! The nurse also tells me that melanoma is a very slow moving cancer but from what I have read it doesn't seem to be so slow moving to me! I need answers because I am going nuts! My primary has put me on xanex because my anxiety has completely shut me down! When you hear the word cancer you have no clue how long you have left and the thought of me being put on a timeline terrifies me. I have good days where I believe in my doctor and think I AM going to pull through this but I just have this sick feeling in my stomach because the lymph node dissection could not even pin point it as melanoma the first time so they sent it to Pittsburg where they said that it is consistent with melanoma and I wonder if it was really even that! They could not say it was definite just consistent !I really need some support because I just want to feel like me again and every time I start to I dissect what the nurse has told me and I get terrified! It's hard to be strong in front of my husband and kids but I HAVE to be so they don't get scared !

Listen to your doctor and ignore the nurse.
She doesnt know what she is talking about.
In your situation with a microscopic amount found in one lymph node you are technically stage 3 but with a good prognosis.
I doubt you need to do anything other than wait and have regular ultrasounds to check any further lymph node involvment.
I wouldnt advise having any other nodes taken out but some people might disagree but the prognosis for people with micro mets in one node are the same as those who have clear SNBs.
THere are quite a few stuides on this if you want to take a look via google.

She doesnt know what she is talking about.
In your situation with a microscopic amount found in one lymph node you are technically stage 3 but with a good prognosis.
I doubt you need to do anything other than wait and have regular ultrasounds to check any further lymph node involvment.
I wouldnt advise having any other nodes taken out but some people might disagree but the prognosis for people with micro mets in one node are the same as those who have clear SNBs.

I was going to ask you what country you lived in, and then you mentioned Pittsburgh. All of this sounds very, very odd. I'd suggest you look for a new clinic for a second opinion. It would be totally worth it for your emotional (and physical) well-being.

I live in Ohio and I go to Wheeling hospital for my melanoma. My husband and I found out (through this site) that UPMC cancer center is only 72 miles away and we are going to see about a transfer ! My current office has only given me the PET scan results and the nurses have only told me what my pathology results are ( I have NEVER seen them) ! I so appreciate you taking the time to reply !!! I am going to take your advice!!!! HAVE A VERY MERRY CHRISTMAS !!!!!

That nurse clearly hasn't dealt much with melanoma. You don't need to be around people like that. The sooner you get seen at UPMC, the better.
From your description, it sounds like it hasn't spread beyond that one lymph basin so that should put you at Stage III A, B, or C. But I'm no medical professional. Hopefully a visit to UPMC will clear things up for you.

Obviously I agree with what all posters have said! Get yourself to a melanoma specialist! And it sounds as though you are working on that. Here are some reasons: http://chaoticallypreciselifeloveandmelanoma.blogspot.com/2013/02/got-melanoma-get-yourself-melanoma.html
You will probably, among many other things, need to talk to your melanoma oncologist about whether or not you should undergo complete lymph node removal of the area around the positive sentinel node. That is clearly a personal question and one that is not without controversy. You do need to get another opinion from a good place that specializes in melanoma. Here is great site that when you click on your state will give you a list of melanoma centers, makes it very easy!

Hi Rose, sorry about your diagnosis. Just wanted to answer you because I also had a melanoma on my thigh. Stage 1b until they found Micromets in my sentinel node….now stage 3a. I elected for the inguinal pad of nodes to be removed and they were all clear. With a clear PET and Micromets in sentinal….that's 3a. I understand about your anxiety because I also had it that bad after my initial dx. Things have calmed down but still get anxious around scan times. Good luck with whatever else you decide to do!

Everything I'm reading on your post leads me to think you are likely 3A. I wonder if this could be Clark Level 4? Actual stage 4 would be mets to another part of your body from the primary. Try to speak directly to your doctor or perhaps get another opinion/evaluation at the Cleveland Clinic, Mayo or another cancer center with specialization in Melanoma. Good luck!!
