There is no established protocal for CT scans in fact some don't do them at all – most common seems to be every 6 months but it varies according to the onc. 

Regarding upper adoman pain it could be so many things it is hard to rule out.  You need to see your G.P. then go from there.  See this link:

a look at your profile, and see that you were diagnosed with a 3.15mm nodular melanoma last
October. I don't want to worry you, but nodular melanoma can be fairly aggressive in some
cases and one needs to be especially vigilant.

The source of your abdominal pain needs to be investigated, and I feel that a CT (and
possibly a PET) scan should be done as soon as possible. It might be something very minor and unrelated to melanoma, but it is impossible to obtain an accurate diagnosis without scan(s)
being done.

I'm not sure about 2A but the specialist want me to get checked/scans every 3 months for 2 years.  I too had nodular 14mm & ulcerated 2C  so I'm not sure if it's the same protocol.  My mitotic rate was 4mm and they said that was high so yours was pretty agressive.  Even at every 6 months you would think that your close enough to get your scans especially if your having pains.  I'd call my doctor and see.  I totally know how you feel.  Every ache, pain, cough, bump, or whatever is melanoma related.  I drive my doctor crazy but that's what they get paid for!!
