Average time to response is 3 months. Sometimes lesions may even enlarge before shrinking due to influx of t cells.https://www.ascopost.com/issues/june-10-2018/pembrolizumab-in-metastatic-melanoma/ – “the median time to overall response was 3 months”

It was definitely three months for me. My largest tumour was 9 cm, and it was causing a lot of pain. That pain got worse at first, then less. By my first scan at 3 months it was 4 and a half cm! -

Dear caman,My father was on keytruda for three years. His visible met was on his back, really huge (half a tennis ball) and we noticed it getting smaller almost immediately. In three months it was almost gone (just some minor suv activity detected). It also worked beautifully on his lung mets.
So, i am not surprised if you see results after a week, because it happened to us.
I wish you all the best in your fight and hope that you get the greatest of results too!!
Love,
Patrisa
