Seeking advice from the folks who have failed ipi/nivo . My mom originally diagnosed with vaginal melanoma failed nivo in August as disease progressed to liver with two Mets in liver 1 cm. Started ipi/nivo in September and had to stop after her third infusion due to GI side effects
Scan today revealed the tumor grown to 7 cm and multiple new lesions in liver. I have an appointment tomorrow to discuss the plan going forward.Can anyone suggest what are the options after failed ipi/nivo ?
FYI she had all the side effects but still it failed.
She is being treated at Ucsf by dr.Adil DaudStill thinking how do I talk to mom about this. Can’t see her breaking down.

Hi Dear,First of all Hugs!
My mom is also in a similar situation,
We are starting a traditional chemo therapy, followed by Keytruda (also an anti-pdl1) like opdivo.Our oncologist thinks that chemotherapy creates sort of tumor inflammation, and would enhance the response to another immunotherapy. He believes that chemo it self can also triggers solid regression on its own (not a huge probability, but we still would take it – who knows)
Meanwhile, I am searching clinical trials, I am particularly interested in TIL, some of our fellows here have experienced wonderful durable results.
Furthermore, how is the rest of her genetic profile?
She could still benefit from a targeted therapy that focuses on a non-melanoma deformed Gene – for example if she has the gene for breast cancer, she could benefit from a breast cancer targeted therapy, you may want to discuss that too with your doctor.I would also recommend sending a direct message to our friend here MelanomaMike, or read his treatment history for ideas, he has almost tried every treatment on this planet, and he is doing very well inspiring all of us.
Finally, I know what you mean, do not tell her now, go ahead explore some options and talk to her doctors then get back to her with a good idea or two along with the (not so good) news.

I think it may be too soon to say that the combo has “failed.” Is that what the doctor called it? The combo can often some time to work and often continues to work after it is stopped.
