My husband got diagnosed with "lentigo maligna" from a punch biopsy.
Hello,
My husband got diagnosed with "lentigo maligna" from a punch biopsy.
After a wide local excision, the final diagnosis is "lentigo malignant melanoma". The pathology report says clark level 1 with all the cells confined to the epidermis but our doctor says it is not "in situ", but a thin lentigo malignant melanoma, with high metastatic potential. One doctor says it is the same result as the punch biopsy, and another doctor says it is not the same. Another doctor says pretty much all the white people have one of those on their face. They all seem to be confused and now fed up with my questions.
I have spent huge amount of time searching on the internet, but I can't find out what it means. We live in Korea where melanoma is very rare, especially the lentigo maligna melanoma. No doctor seems to have any experience in this particular area.
Is there a way to have our slides reread and get an expert opinion on this, possibly in Australia or in the United States? preferably without having to fly there? (We have nobody to take care of our 4 year old) We are not citizens of either one of the countries…

Do you have the full copies of the reports?
Lentigo Maligna Melanoma doesn't usually have "high metastatic potential". And if it is thin or in situ, that makes even less sense. As I stated before to you, I think, it can have a high LOCAL recurrence rate which is not the same thing. Yes, many white people have "lentigos" or age spots as they get older, but that doesn't mean they are Lentigo Maligna.
Yes, you can request the pathology slides and just have those sent elsewhere. No need for you yourselves to travel. Basically, if you can obtain the slides, then I'd just contact a major melanoma center in Australia – or in the US – and have them shipped there for another opinion.

Why not contact these people? They have pathologists and certainly know a lot about melanoma, being in Australia.

Thank you so much. I always get lots of help from your comments. You seem to know a lot more than most doctors, certainly the ones who are not melanoma specialists. Thank you for helping people you don't even know and you haven't even met.
I do have the full copies of the reports and we got the slides, too – initial biopsy, post surgery (one stained, one not stained whatever this means.)
Now that I know it's possible, I should just proceed with it. Any institution that has expertise and experience who is willing to read slides of people from long-distance is what we are looking for. You put all these into perspective. Thank you so much.
