    I will first say that I will be calling my onc's office in the morning if I still feel this way, and will email if I feel better but still want to let them know.

    Just wondering for others who have been on or are on the Ipi/Nivo combo and have experienced low grade fever, body aches, and chills…. did you find a cause, if so what was it? How long did it last?

    I had my 4th dose on 12/29 and have never felt this before. Appetite was gone yesterday and today, I've had some on and off appetite loss throughout the combo, so that part is not new. Now I have a low grade fever, 99.2 last time I checked and my body aches pretty bad and chills are not fun either. I got a flu shot a couple of months ago, so I don't think I have the flu.

    My labs last week showed very slightly high white blood cell count and slight anemia. Onc wasn't concerned. She asked me if I had been feeling sick and I had not.. until now.. that was 5 days ago.

    Any insight would be great, thanks!

      This actually happened to my mom twice after 2 of her combo treatments. She ended up feeling very fatigued, cold, chills and wasn’t able to eat, she also had pain in her hips and bones like she had never felt before. She had a fever of 100.5 the one time. When we asked the Dr what it was, he said it was the immunotherapy (I also remember reading these came as side effects in a packet he gave me) and that if she didn’t have a fever over 101.5 to not worry. He didn’t seem alarmed either time we told him and nothing was prescribed to her.

      She was sick for about 1.5 days (slept a lot during this time) then on the 3rd day after she was fine. We never did figure out what it was from. She had elevated WBC as well, but low protein levels no anemia.

      Best advice would stay hydrated (that’s about all my mom could do as well since she had no appetite) and just rest. Call your Dr to make sure anyway since everyone’s situation is different. Hope this helps a bit!

      Just took my temp again: 100.6 now

      I can count on one hand the amount of times I have had a fever in my life, so this is weird to me, these body aches suck!

I can’t even say I would recommend Tylenol, my mom tried it and it didn’t do a thing for the aches or fever.         Another update: 101.2 now

        Took more Tylenol since it's been a few hours since I took it last. If it doesn't help and fever goes up more then I may need to take a trip to the ER.

      My Mon experienced this will ipi. Was in the ER and hospital twice. They couldn’t find anything. All she needed to do was take Tylenol if it started and she was fine. If this doesn’t work call your doctor.

        Thanks Patina, I took Tylenol a couple of hours ago, will take another one in a little while and see if it does anything.

      I experienced some of the same side effects immediately after my second ipi/nivo infusion (and my third overall infusion, since I had started with a single dose of nivo). For about a month, I was running a pretty high fever, around 101-02, that we kept in check with a steady stream of ibuprofen and tylenol. Also, I was extremely fatigued throughout the month, had constant chills and headaches, had very little appetite, and mostly kept to my bed or couch.

      Prior to getting my third combo infusion, my bloodwork showed elevated liver enzymes (about 5x ULN). My doctor decided to postpone any further treatment and instead started me on a pretty heavy daily steroid regimen. I felt better almost instaneously and these side effects quickly subsided (although others lingered for quite a while longer).

      I think the thinking is that, in my case, all of the side effects, including the fevers and hepatoxicity, were immunotherapy-related. Fortunately, while going into overdrive, my immune system seemed to have destroyed a lot of cancer, too.

      By the way, I was treated at UCSF. As I'm sure you have, I received very attentive and responsive care from the physicians and staff both during and outside of working hours. So don't hesitate to get in touch at any time because, at the very least, there will be an oncologist on call to handle side effect concerns.

        Thanks for the info.

        I am waiting for the on call doctor to call me… it's taking longer than I'd like. Fever went up to 101.6 and then an hour later down to 100.4.. so, who knows, but hopefully I can talk to someone and get an oncologist opinion on this.

        In the ER since on call doc said it’s better to be safe than sorry. Fever has gone down and I’m feeling less achy than earlier. Getting blood work done and have IV fluids in now.

      Around my third dose of the combo I started running fever and it lasted 6 weeks. Spent several days in the hospital at first running every test for infection, nothing. Doc finally agreed it was a side effect. It was the worst chills I have ever had. I would sit in my hot tub in july shivering. I learned that If I took motrin every 2 hours the fever stayed under control. If I stopped to see if the fever came back it would and then I would go through the whole cycle of chills and sweats. I was even sleeping on a cot with a plastic pad with a stack of sheets beside me. During the night I would soak a sheet, discard it, slap a dry sheet on. Lasted about 6 weeks and have not had any since.

        That sounds awful, but I'm glad it wasn't from something more serious going on. Thankfully the chills have subsided and I'm left with a very achy body… kinda feels like I got hit by a car. Hopefully this doesn't last too long for me. Had a bunch of blood work, cultures, and other tests run at ER, all came back normal which is good.

        Hey Jenn,

It was thanksgiving weekend and I called my onc that evening, he told me to just to take ibuprofen and call if it got worse. They had taken a bunch of blood cultures and UA before my infusion and everything came back normal. The fevers continued for a week straight. the highest it got was 103.2. Just like one of the other posters said I would sleep with towels and blankets under me and have to change my clothes/sheets like 3 times a night. I could control the fevers with ibuprofen but they would come right back once it wore off. I would have shaking chills and then my fever would break and I would sweat like crazy. I did not lose my appetite which was strange. Eventualy after 5 days of this I told my onc and he finally told me to go to the ER. They ran a bunch of tests and everything came back normal except my platelets were low (92,000) and I was mildly anemic. They stupidly didn't run a liver panel. I went and saw my onc the following monday and my LFTs were 2x the normal limit.  They started me on prednisone and the fevers went away immediately. They thought I had autoimmune hepatitis (since 2 days later my LFT's were still trending up even with the prednisone) and I had a liver biopsy with mixed results (liver toxicity injury vs autoimmune hepatits). After being on 60 mg of prednisone for a week we did a fast taper and I finished it 2 weeks later. Everything was normal. That was 2 weeks ago. We discontinued the Ipi after my 2nd infusion since i have NED after surgery and the side effects were terrible. However last night I had a fever of 102.9 again. Not sure what's going on but going back to MGH today or tomorrow. Make sure they check your LFTs! Hoping I don't have to start prednisone again. Best of luck to you!

      I've had about 20 nivo infusions (including the combined ipi-nivo infusions).  I also was on pembro in 2015.  Every few weeks or so, I have a low grade fever (including while on antibiotics).  Usually under 100.  Comes and goes and I manage it with Tylenol.  I agree that a trip to the ER makes sense for a higher fever like yours.  Good luck.

      I've had about 20 nivo infusions (including the combined ipi-nivo infusions).  I also was on pembro in 2015.  Every few weeks or so, I have a low grade fever (including while on antibiotics).  Usually under 100.  Comes and goes and I manage it with Tylenol.  I agree that a trip to the ER makes sense for a higher fever like yours.  Good luck.

      Jenn, hope it was just a cold or flu or something like that

      had same thing two weeks ago (am on pembro) but this was not the pembro just a sore throat and cold

      i also dont usually get fever so was surprised and emailed my doctor (no ER)

      am a little surprised you take tylenol and not ibuprofen: i tend to get bad liver readings and my doctors prohibit tylenol (alcohol too!) – but your liver readings seem ok

        Thanks Mark. I don't usually take Tylenol, but my doc said it was ok to take to keep fever down. Liver tests are normal so no worries there. Hope you're feeling better after your cold.
