Hi,
I'm interested in stories or research showing whether a tumour rupture helps the immune system detect future melanoma's and improves prognosis or whether it causes metatasis and spread and worsens prognosis.
I can find some research that shows other types of cancer spread and worsen, and some research that shows melanoma has an immune response rather than spread, but I was hoping to balance it out to see whether there is a pattern or whether it's just random. Any survivor stories of ruptures appreciated too.

I'm not sure about rupture of a melanoma tumor, as say in a traumatic injury. Mel tumors tend to be very hard, and I imagine it would take a lot to rupture them. Also, since its not unusual for doctors to perform, biopsies of tumor material, I suspect they know the risk of spread through that damage is minimal. One thing I think is clear, a typical immune system is no match for melanoma. It is with the added benefit of immunotherapy drugs which suppress the tumors natural defenses that allow the immune system to penetrate the tumor and attack. Check Wikipedia for a simple explanation of the "abscopal effect". This is where radiation begins to destroy a tumor, and it releases antigens which the enhanced immune system then targets, and in many cases the other tumors in the body creating the hoped for systemic response. This is the only scenario I'm aware of where tumor damage can lead to aggressive immune response. Others here may know more.

I have never heard the term rupture used by any Oncologist to date when talking about Melanoma treatments. Are you possibly talking about antigens being released by melanoma tumors and the immune system responding by stimulating t-cell production which is the basic principal of having ones immune system fight the cancer? Dr.Allison of M.D. Anderson in Texas, one of the leading research guys in Immunotherapy and one of the key scientist behind Ipi(CTLA-4 brakes on the immune system) has talked about tumor cells releasing bits of dead tumor. Dr. Allison talks about how the immune system builds memory and if any more melanoma with the same antigens show up that they will be attacked. Is this what you are looking into? Ed

You might want to look up Laurie's (sweetaugust) profile and posts. She talks about a met node that blew up and died after she started treatment with pd1 MK-3475. Maybe she'll chime in, but I believe she's doing well now.
