Evening everyone. I have 2 questions tonight if anyone could give your inputs and experiences.

      Quick Bio.

      Diagnosed back in December.

Had Shave biopsy Nov 27th, 2013. Pathology results said "Surgicial incision with 1 cm MA of skin. Left thigh. Superficial Spreading Malignant Melanoma. Level 2 invasion, vertical height 0.47- No ulceration. Mitotic index is one mitosis per 1 mm squared."

WLE Dec 31st. Pathology results appointment Jan 29th. Feels like it will never come!

So on January 10th lymph nodes in groin were tender and felt bumps in a few spots on left side. Went to family Dr on Monday the 13th. He felt it and said that he doesn't think its a tumor but said that he felt a few and it could be because of the WLE and is just fighting the possible infection I could have. However he couldn't tell because of all the bruising which is huge. Not hot to the touch so he said play it by ear for now and should it get warm to go and see him and he will write a prescription. Still breast feeding my 6 month old so happy he doesn't think its infected and I need to go on anything and have to stop. He did however want to have an ultrasound done on my lymph nodes to check for sure. I have an appointment on the 28th of Jan for that. Swollen Lymph nodes are still there, however, they are not tender anymore like they used to be.

So first question. Has anyone ever had a ultrasound to check lymph nodes? I can't find anywhere on the web where it says that one would use an ultrasound to check lymph nodes.

Second question. I have my first full body check on Thursday and I would like to hear other peoples experiences and what to expect. I am already angry at my Derm for doing a Shave Biopsy(Thats all they do there) and I want to make sure this time I am informed going in as last time I just went along with what they said. My family dr is going to set me up with someone else, in addition to my derm. He had training in Australia for Melanoma "detection"(Not the proper term) So I can have two sets of eyes which will be nice. Is there anything I should make sure they do?

Any answers are greatly appreciated. Many thanks.

Hi, well it sounds as if it was caught quickly enough.  Though it would be better with zero mitosis, still, under .75 only level 2, that is a pretty good place to be if you have to have Melanoma.  Yes, I have heard that some MDs do use ultrasound to check for enlarged nodes. Especially when your stage is not advanced, they will not usually do CT/ MRI / Pet scans because it's cost prohibitive and they don't want the patient exposed to so much radiation.  Too bad about the WLE, I had 2 cm taken, hardly any bruising and very little pain, no infection.  So yes, your body most likely is having an inflammatory response to the surgery, but it's good to do the ultra sound to follow up. Good Luck!

Thank you for your input Momrn5! I am glad  to hear that you heard of them doing ultrasounds. It's going to be strange having a ultrasound and checking the lymph nodes oppose to a baby this time around. As for my WLE. It is just in a bad spot (well every spot is a bad spot right) so thats why it bruised so bad. It is getting much better now and doesn't look so bad. I don't mind it at all now except for when I hit it by mistake or my 6 year old comes and gives me a hug and gets me everytime in that spot. Worth it though for a hug! So can't wait for next week. Hoping there are just reactive.

I get my first 3 month check up tomorrow. I have a few concerning ones in my opinion so fingers crossed that they are just ugly freckles. I have so many freckles and new ones appearing all the time should be intereresting.

                  Thanks again!

                      Hi Yvonne,

I've gone with my husband twice when he had full body checks. The first was with his dermatologist, who removed the "cyst" that turned out to be a big primary melanoma – not visible on the skin, so it's hard to fault him for not finding it. He visibly scanned every inch of my husband's body and froze off some precancerous lesions. He has also done shave biopsies a few times, and nothing has turned out to be melanoma except the big messy "cyst." He is not very receptive to suggestions for other ways to deal with questionable spots, and my husband has not pushed him to do anything differently.

The other visit was with a skin cancer specialist at Johns Hopkins, whom we consulted during one of our visits there for a clinical trial. He showed me different spots – told me which ones he was concerned about and which ones not. It was a good education for me, and reassuring, but we haven't gone back to him.

I don't like my husband's dermatologist here in town because he wasn't willing to give me the education that the Hopkins doc was. Also I probably have some residual concerns that he didn't diagnose the original "cyst" properly, but that's probably unreasonable since it wasn't a typical superficial spreading melanoma lesion.

I'm not sure how it is in Canada, but if your primary doc can set you up with a different dermatologist for future checkups, that would probably be better for you. Also it would seem that someone could give you the results from the WLE sooner than Jan. 29 – that's a long time to wait! We had word of pathology reports a little more than a week after the surgery.

In general, though, all that waiting for results is nerve-wracking. I wrote about it a lot on my blog in the early days of Robert's treatment. One thing you'll see there is that we were fortunate to have enough "good" doctors feeding us information to counter-affect the frustration we had because of those who didn't communicate with us as well.

Please keep us posted! Wishing you clear margins and good scans!

                      Hazel

Thanks for your link to your blog, as well for your well wishes!

Little update posted at the bottom of the page.

I agree that waiting this long is crazy. My family dr called to get them and was denied. NO lets make her sweat it out says the secretary. Grrrr. I had my initial biospy results 7 days later. Just ridiculous. Thank god its almost here.

I questioned the derm at the appointment in regards to shave vs punch and she was saying that shaves get the correct depth and kept coming back to it after I asked her about it. Like she was defending herself. It was strange. Then said I qualified for the SNB with a mitosis of 1 and that I should of discussed it with the surgeon. And came back to that twice. Weird appointment.

                          Keep you updated for sure.

                          Thanks!

Trauma can definitely cause lymph nodes to enlarge, and I think you can consider a WLE trauma.  Certainly ultrasound is used for monitoring lymph nodes – there have been clinical trials for that very purpose:  Ultrasound vs SNB.  So that is valid although I'd say overly cautious at this point.  You have a very low risk lesion and it just needs to heal!

As for a body check, it's basically looking for that ugly duckling.  Since most warriors NEVER have more than one melanoma primary, it's just about monitoring your skin for change.  Personally, I prefer photographs that I can use as a baseline as opposed to more eyes.  If something isn't changing, I don't remove it.  But if it changes, it's gone.  Photographs are an easy way to look for change.  I'm not into unnecessary biopsies and the photographs are my baseline.  More eyes kind of muddles the picture – all I want to know is that a lesion is stable (leave alone) or changing (biopsy) and that's it.  I have found my 3 primaries and really don't expect my doctors to find one on me.  I find a suspicious mole, my doc and I confirm it's changing from the photos and then my doc removes it.  That's how I approach living with melanoma.

Hi everyone. Thank you all for your replys, thoughts and inputs!

2 days till my wle report. Anxious for sure.

Full body was done. Derm wants 5 more biopsys done. 2 on my stomach, those were 2 I was worried about because I didn’t have them before and they are in a easy spot to notice. I let her check me out with her gadget before I said anything and she pointed them out. 2 on my back. 1 on my right leg. She said we should biopsy them as long as I don’t care about the scars. She wants to do them in 2 sessions.

I have my appointmemt with my surgeon wednesday so I am going to talk to her about it. The other derm I mentioned, I am going to see if my family dr can set up a appointment at least to get a second opinion on these.

I mean if you were in my shoes would you have these 5 biopsys done if they suspect something now or wait a little longer to see if they change more? I like Janner’s approach to the pictures and you make the calls. I noticed the 2 on my stomach and have pictures that they weren’t there before, so I am scared to not get it done as I seen what a mole that pops up turns out to be. Not every new freckle or mole will be Melanoma and I guess I have to make a decision at what point do I make the biopsy call, before I end up looking like a teddy bear sewn back together after a dog got a hold of it. The what if’s suck in this mind game disease.

Biopsys were scheduled for Feb 4th and 18th. Which would be shaves. Another thing I want to ask my surgeon. If they really want to get these biopsied, can I get punches done instead by her.

Does anyone have any advice? Is this lady just trying to shave me up because she can? I am not sure how these derms get there paychecks in Canada but if its by appoinments, I can see my schedule book filling up pretty fast. Think its time to head to Toronto to a Melanoma specialist hospital.

Very wise advice, Janner. Not only your technique, but your attitude– "living with melanoma".
