I realize your father (??? – as I understand it) has been through a really horrible time with colitis and what seems to be a rectal abcess {???}. As awful as that has been, many melanoma patients who react badly (sometimes very badly) to the ipi/nivo combo can return to nivolumab (Opdivo) as a single agent once the adverse immune reaction to the combo has been controlled. We all know that anti-PD-1 alone can cause nasty side effects as well, but for things like your dad has experienced with the combo – ipi is usually the bad boy there. That option is discussed here in the first article of the post: https://chaoticallypreciselifeloveandmelanoma.blogspot.com/2016/05/asco-2016-three-anti-pd1-reports.html
Here are the NEW ta-dah’s in melanoma treatment out of ASCO this year: https://chaoticallypreciselifeloveandmelanoma.blogspot.com/2019/06/new-stuff-treatment-options-and-current.html Some are combined with immunotherapy, some are not.
Treatment options without immunotherapy include intralesional injections (if the patient has a lesion that is superficial enough for that option), radiation, TIL, and surgery. There is a great deal of information on all of these and more on my blog if you are interested. The search bubble is your friend.
However, if I were in your father’s shoes – I would try to rest and get well from the ordeal he has been through, then carefully and cautiously proceed with nivo as a single agent. But, that’s just me. Hope this helps. I wish you both my best. Celeste
