adjuvant therapy?

      Hi,

       

      my sister has been diagnosedwith malignant melanoma in 2009 stage 3. She had 3 surgeries to removed the primary and lymph nodes. After that was receiving chimo, interferon and radiotherapy.

      This august on the last CT scan a lymph node (between the kidney and spleen) was looking suspicious on the image. 

      She had a surgery to completely remove the lymph node (fully encapsulted and tested positive for melanoma). Everything else looks clean. 

      Does she need any adjuvant therapy after sugery?

          If she has already done Interferon, then there really is no other adjuvant therapy available unless she looks into clinical trials.  Right now, only Interferon is approved for stage III.  Is the new lymph node "distant" making her stage IV?  She might have a few more clinical trial options that way, but there just aren't many treatments available for stage III/IV with NED (no evidence of disease).  I'd check out clinicaltrials.gov to see what options might be available.

                Celeste just posted this a little while ago on another post about a stage 3 NED trial. Figured I should post her post here too so you see it. So here is her post from that post but it seems like it should apply to you as well I think:

                 

                I don't know anything about the test you are using.  However, if you are stage III and are interested in taking action…there is an NED trial for folks Stage III and IV combining ipi and nivo available at Moffitt in Tampa, Florida. 

                Here's a description of it from my blog:  "A new cohort #5 has been created.  It will accept 1,500 resected melanoma patients, from Stage III B/C to Stage IV.  Due to the high incidence of side effects with the prior combo….the dosages were flipped.  These patients will now be given Nivo at 3mg/kg and Ipi at 1 mg/kg.  Side effects remain a concern, especially for those at Stage IIIB as their risk for their melanoma is less than that of NED patients Staged IIIC/IV, and such side effects always have to be weighed against risk of disease, but should be decreased for everyone with this particular dosing plan.  There is no HLA typing requirement as no vaccines will be given."

                    Yes. There is adjuant therapy for stage 3b and 3c via a clinical trial now. It has just started, I believe.

                    I don't have the details to hand but it seems to have replaced the combi ad trial with the same drugs and not randomised between placebo and the drugs.

                    I was offered it. they advised take the drugs for three months, have surgery on a suspect lymph node and then continue on the drugs for another nine months. 

                    Further tests showed the lymph node not to be suspect so I am one of the last to go on combi ad. I am sitting here after two doses sweating and hot so unless I am kidding myself I am getting the real thing.

                    If you can't find the information on the trial then pleasemessage me and I will find it for you.
