    Hi Warriors

    Last time I posted it was good news; 30% reduction in liver lesions.  However, it seems the inhibitors are petering out now.  Next best step?  We love team at MSKCC but you guys always get me thinking.  All advice and suggestions welcomed.

     

    Recap;

    Husband is 45 y.o. Irish, blue eyes…raised on the jersey shore (lots of sun exposure).

    July 2010 Melanocytic Nevi removed from left calf. 

    F/U with Derm exams x1/6 mos

    June 2013 Nodules appear under skin on left calf. We thought they were vericose veins.

    July 2013 Swollen left lymph node in groin area. Biopsy.  Melanoma. BRAF WT.

    Slide from 2010 reread by MSKCC as melanoma in situ

    August 2013 Lymphadenectomy of left groin.  Just uppers Cloquet node negative.  5/19 nodes positive.

    October 2013 Start Yervoy

    November 2013 Radiation to Lymph node basin of groin

    January 2014 Prednisone needed to control colitis from yervoy. Genomic Studies show NRAS positive.

    March 2014 End Prednisone

    April 2014  PET/CT Scan shows 3.5 cm lesion in fundus of stomach. Confirmed through biopsy.

    May 2014 start PD1/KIR trial

    July 2014  too much bleeding from stomach tumor.  Surgery to remove. 

    July 2014 Scan shows mets to liver.  numerous.

    August 2014 Start MEK/CDK4

    October 2014 30% Reduction in liver lesions

    November 2014 20% more reduction

    December 2014 Stable Lesions.  Heart EF low…need to lower doses

    Today Scans…new Liver lesions.  Subcutaneous lesion on back.  Maybe on ribs.

     

    Options: checking HLA-A2, if positive candidate for IMCgp100 (Monoclonal T Cell Receptor anti-CD3 scFv Fusion Protein) IMMUNOCORE trial at MSKCC.

    Options: Speaking with NIH nurse tomorrow about IL2 trial eligibility.

     

    We did a big tumor genetic profile test of in September at MSKCC.  Still waiting for results.

     

    What would you do?????

      Sorry you and Mike are going through this. Is oncologist at MSKCC recommending IL-2? Do you see Wolchok?

      Anyway, I did it and it's tough but very doable. I know there are some out there who swear by it. Also if it works, you know fairly quick. I did a combo trial of ipi followed by HD-IL2. I was getting good response from ipi right away and scan 2 weeks after I finished IL-2…lung nodules gone.

      When you speak to NIH about IL-2…is this just IL-2 or TILTheraphy? That may be an option.

      Sorry you and Mike are going through this. Is oncologist at MSKCC recommending IL-2? Do you see Wolchok?

      Anyway, I did it and it's tough but very doable. I know there are some out there who swear by it. Also if it works, you know fairly quick. I did a combo trial of ipi followed by HD-IL2. I was getting good response from ipi right away and scan 2 weeks after I finished IL-2…lung nodules gone.

      When you speak to NIH about IL-2…is this just IL-2 or TILTheraphy? That may be an option.

        Yes…Dr. Wolchok is our guy.  He is giving us both options.  Are you NRAS mutation positive?  Did you do IL-2 down at NIH?  What were you side effects? 

        So much to think about.

        I'm BRAF wild….though I don't think I was tested for NRAS. I did IL-2 in Chicago…Lutheran General Hospital under care of Dr. Richards & Hallmeyer. Like I said…rough but doable. Again, I did this in combo with ipi. We knew pretty quick that I was having response to ipi so and by time I finished IL-2 I was NED…did the IL-2 help? I'd like to think so….but not sure. I also just had a local recurrence. I never had clean margins brfore I started treatment.I think the thought was that treatment would resolve cheek area.

        Mike just returned from visiting with Rutgers.  The physician said he could start IL-2 as early as next week. We are still waiting to see what NIH has to offer. And we want to see what MSKCC has as well. Deep Breathes. It just seems you have to make the best choice in the best sequence.  Uggggh.

        Further update:

        Unfortunately, he now has two little brain mets. We are so scared. So many questions. If we read correctly, the phase 1 of Immunocore show a 13 percent response. But our oncologist really wants us to try it over IL2 or chemo. Plan B is Abraxane/Avastin with gamma knife for brain mets. 

        we are bouncing things off the group here.  What would you do?
